• Deliver evidence-based, quality care to each patient enabling them to achieve their prior level of functioning or progress to the next level of care.
• Establish timely treatment plans tailored to each patient’s needs.
• Demonstrate a positive connection with patients, families, clinicians, and hospital staff.
• Build and maintain relationships throughout the hospital with physicians, case management, nurses, and other team members.
