Job summary

Evolving Solution Services is seeking a full-time APC to provide comprehensive medical care to SNF residents in California, operating under California licensure and facility privileges.

You will perform evaluations, manage plans of care, prescribe medications within scope, document in the EMR, participate in interdisciplinary care, and cover on-call shifts as assigned.

This role requires collaboration with nursing, pharmacy, and supervising physicians to ensure high-quality patient outcomes.

Qualifications

• Must maintain a current and active Nurse Practitioner license in California.
• Must maintain a current and active Physician Assistant license and related certification as applicable.
• Compliance with licensing, credentialing, and scope-of-practice requirements is required.

Responsibilities

• Provide comprehensive medical evaluation and ongoing management of SNF residents within California license scope.
• Develop, implement, and update individualized treatment plans and orders.
• Prescribe, furnish, and/or order medications and treatments as permitted by state and federal law.
• Maintain timely medical documentation, progress notes, assessments, orders, and treatment plans.
