The ideal candidate holds an MSW from a CSWE-accredited program and is a member of the National Association of Social Workers (preferred). This position serves patients and families through clinical social work and resource brokerage in a hospital setting.

Formación

• MSW from CSWE-accredited program required.
• Active NASW member preferred.

Responsabilidades

• Advocate for patients, acting as liaison between patient and medical personnel.
• Address emergent needs through counseling, education, and brokering community services/resources to support long-term health and well-being.
• Provide clinical social work, case management, and discharge planning within the continuum of care to ensure efficient, compassionate care.
• Develop and implement treatment plans addressing patient-specific needs and coordinate with health care team.
