Broward Health in Fort Lauderdale seeks a skilled Case Manager RN to coordinate a caseload, promote evidence-based clinical outcomes, and support discharge planning in line with regulatory standards.

The role requires an active RN license and Basic Life Support, with at least two years of acute care experience. An associate degree and nursing program accreditation are preferred for this full-time position.

Qualifications

• Two years of acute care experience.
• RN licensure (RN-LIC) required.
• Basic Life Support (BLS) required.

Responsabilités

• Facilitates, coordinates and manages an assigned caseload to promote evidence-based clinical outcomes.
• Assesses need and coordinates safe and appropriate discharge planning in compliance with state/federal requirements and accreditation standards.
• Supports medical necessity determinations for appropriate level of care and resource utilization.
