Job summary

Broward Health in Fort Lauderdale seeks a skilled Case Manager RN to coordinate a caseload, promote evidence-based clinical outcomes, and support discharge planning in line with regulatory standards.

The role requires an active RN license and Basic Life Support, with at least two years of acute care experience. An associate degree and nursing program accreditation are preferred for this full-time position.

Qualifications

• Two years of acute care experience.
• RN licensure (RN-LIC) required.
• Basic Life Support (BLS) required.

Responsibilities

• Facilitates, coordinates and manages an assigned caseload to promote evidence-based clinical outcomes.

Summary

Facilitates coordinates and manages an assigned caseload to promote evidence based clinical focused outcomes. Manages utilization management supporting medical necessity determine appropriate level of care and resource utilization and evaluates the need for services for continuity of care and denial prevention. Assess evaluate and coordinate for safe and appropriate discharge planning needs in compliance with state and federal regulatory requirements TJC accreditation standards and policies.

Retention Bonus: $10000 - 2 yrs of experience required
