Job summary

Atlantic Health Morristown Medical Center in Morristown, NJ, seeks an experienced Nursing Case Manager to coordinate patient care across the continuum, lead multidisciplinary rounds, and optimize discharge planning.

You will assess, plan, implement, and evaluate services with the Case Management team to maximize quality of care and efficient use of resources; NJ RN license required; acute care background preferred.

Qualifications

• Bachelor's degree in Nursing.
• Valid New Jersey Nursing License Required.
• Minimum of 3 years of Acute Care experience preferred.

Responsibilities

• Manages, coordinates, and facilitates the care delivered to patients through patient/family assessment and multidisciplinary collaboration.
• Ensures daily chart review, utilization reviews, coordination of discharge planning activities and documentation.
• Provides interdisciplinary patient care rounds and addresses quality of care barriers and discharge delays.
• Formulates and implements discharge plans to ensure safe, timely patient discharge.
• Works with the healthcare team to manage the patient care plan.
• Performs variance tracking, outcome analysis and identifies problems to improve pathways.
• Represents the department and hospital community outreach.
• Performs other related duties as assigned.
