Job summary

Easterseals PORT Health (ESPH) is seeking a compassionate Housing Specialist to join our Assertive Community Treatment (ACT) team in Alamance County. The role focuses on securing and maintaining safe, stable housing for individuals living with severe mental illness and helping them build skills for long-term community success.

You will work on housing stability, tenancy support, and rehabilitation services within a multidisciplinary team, serving Burlington and surrounding areas.

Qualifications

• Caseload management and coordination with the ACT team.
• Ability to develop and monitor individualized treatment plans.
• Experience supporting housing stability and tenancy supports.

Responsibilities

• Provide case management for an assigned group of individuals, coordinating care with the ACT team.
• Develop, implement, review, and revise individualized treatment goals and service plans.
• Deliver rehabilitative services including housing and tenancy supports, money management, ADLs, and wellness activities.
• Build and maintain relationships with housing stakeholders and providers.
• Coordinate communication with guardians, care coordinators, landlords, and community agencies.
• Provide individual or group supportive therapy and symptom management.
• Conduct comprehensive assessments of psychiatric history, substance use, health, and housing needs.
• Participate in crises support and on-call rotation; educate families and advocate for client rights.
• Document services and progress in the electronic health record; participate in team meetings.
• Support clients during daily operations and in the transition to community living initiatives.

In this role, you will support individuals living with severe and persistent mental illness in securing and maintaining safe, stable housing while building the skills needed for long-term community success.

Your Role in Our Mission

As a Housing Specialist, you are an integral member of the multidisciplinary ACT team. Your work focuses on housing stability, tenancy support, and rehabilitation services, including individuals participating in the Transition to Community Living Initiative (TCLI). You will be serving the Burlington and surrounding areas.

Key responsibilities include:

• Providing case management for an assigned group of individuals, coordinating care with the Individualized Treatment Team (ITT)
• Developing, implementing, reviewing, and revising individualized treatment goals and service plans
• Delivering rehabilitative services, including:
  • Housing and tenancy-sustaining supports
  • Money management and entitlement assistance
  • Activities of daily living (ADLs)
  • Health, wellness, recovery, and community integration
• Building and maintaining relationships with housing stakeholders, including:
  • Housing Authorities
  • Permanent supportive housing providers
  • Rapid re-housing programs
  • HUD-VASH
  • NC Housing Finance Agency
  • Local landlords
• Coordinating communication with guardians, care coordinators, landlords, and community agencies
• Providing individual and/or group supportive therapy and symptom management interventions
• Participating in comprehensive assessments including psychiatric history, substance use, physical health, social functioning, and housing needs
• Supporting individuals during crises and participating in an on-call rotation
• Educating and supporting families while advocating for client rights and preferences
• Documenting services and progress accurately and timely within the electronic health record
• Participating in daily team meetings, treatment planning, and organizational reviews
