The Activity Lead is responsible for development, implementation, supervision and ongoing evaluation of the activities program. This individual provides completion and/or directs/delegates the completion of the activities component of the comprehensive assessment. The Activity Lead contributes to and/or directs/delegates the contribution to the comprehensive care plan goals and approaches that are individualized to match the skills, abilities, and interests/preferences of each resident.
JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.

EXPERIENCE
Minimum Required: None
Preferred/Optional: One or more years of experience in a health care patient activities program setting working with elderly populations preferred
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Must have a Certified Nursing Assistant (CNA) certification awarded by the State of Wisconsin within six months of hire and maintain the certification once it is awarded. Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire. State of Wisconsin driver’s license with an acceptable driving record.
Preferred/Optional: Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) at time of hire.
