Job summary

Missouri Slope is seeking an Activity Program Assistant to support the Activity Director in coordinating and carrying out activities for residents. Responsibilities include managing MDS data, completing care plans, and assisting with the Day Program.

You will help with scheduling, training volunteers, and data collection for QAPI studies, while promoting compassionate, respectful care and adherence to facility policies.

Qualifications

  • APND membership is required or obtained within a set amount of time.
  • Previous activity program experience in long-term care setting preferred.
  • Must pass drug test and background checks including criminal history and credential registries.
  • ND CNA certification current or obtained within a set amount of time.

Responsibilities

Starting wage: $20/hour, with a $1 increase with CNA Certification!

8:00 am - 4:30 pm | M - F | 40 hours/wk | Rotating weekends* and holidays**

Activity Director

PRIMARY JOB FUNCTION

Assists Activity Director in coordinating and carrying out activities on assigned units. Completes the MDS process on assigned residents. Assist with the Day Program as assigned. Provides support services to the activity assistants in ordering supplies and providing materials as needed for activities

ESSENTIAL JOB FUNCTIONS
  • Assist the Activity Director and Assistant Activity Director with the MDS process including gathering data, establishing specific goals, developing an activity care plan, and completing electronic documentation. Will complete care conference reports and attend care conference meetings as assigned.
  • Will need to understand and follow Activity Guidelines as outlined by the state and federal regulations under the supervision of the Activity Director.
  • Assist the Director in the daily scheduling and recordkeeping of the Adult Day Program participants within the Activity Program, once the interview process and the care plan has been established by the Director.
  • Will complete 10 hours of continued education annually through APND (Activity Professionals of North Dakota).
  • Assist the Director in scheduling, training, assigning and keeping volunteer records for the department as required.
  • Assist with gathering data for departmental QAPI studies and reports as assigned.
  • Will participate in a team approach in planning a monthly Activity Calendar of events.
  • Will attend department and other staff training meetings as required.
  • Will assist the Director in coordinating services for small sensory groups and residents on hospice care with the Music and Memory Program and IPod Programs.
  • Will conduct self with integrity in all manner of conversing with Activity
  • Assistants, Department Managers, residents, families, and volunteers in a respectful manner. Will uphold the MSLCC Mission and Values.
  • Will be responsible for promoting effective communication among Activity staff related to daily activities and department operations and department policies.
  • Must respect and uphold resident rights in all regards including confidentiality of resident information and refrain from any action that would be considered abusive or disrespectful to residents and willing to hold staff accountable to the facility standards.
  • Will be aware and compliant with all department and facility safe practices as established in the department, and as related to the Safety-Risk Management Program, and the Voluntary Protection Program.
  • Will assist the activity staff in obtaining and maintaining supplies and materials as needed for activity programs.
  • Must demonstrate flexibility and be able to adapt to changes in procedures and sudden changes in assignments as needed.
  • Must be punctual and dependable with attendance.
  • Must be comfortable speaking in front of the department and groups and leading groups and have effective communication and reporting skills.
  • Must have visual acuity and the ability to read, write, verbalize and understand
  • English well enough to receive oral and/or written instructions and successfully communicate with residents and co-workers and complete electronic documentation on permanent medical records using PCC.
  • Hearing acuity must be sufficient to communicate effectively with residents, staff and the public and to use the phone on a frequent basis
  • Must be able to manually lift and push up to 20-30 pounds – up to 30 % of the time during an 8 hour shift. Must be able to transport residents to and from activities as needed and to assist residents on and off the bus for activities outside the facility.
  • Will be expected to lift up to 60 pounds to transfer both residents living in the care Center as well as those in the Day Program on an as-needed basis, up to 20% of the time or less during an 8-hour shift.
  • Will demonstrate MSLCC’s customer service standards of excellence.
OTHER JOB FUNCTIONS
  • Attend scheduled workshops as assigned by the Activity Director to meet continuingeducation requirements for this position.
  • Assist in the performance of resident care as a CNA on a PRN basis as may become necessary in the course of duty within lifting limitations.

Requirements
MINIMUM QUALIFICATIONS
  • APND membership is required or obtained within an agreed amount of time, with continual education to maintain membership
  • Previous work experience in an Activity Program in a Long Term Care setting preferred
  • Current ND CNA certification or obtained within an agreed amount of time
  • Successfully pass a drug test, background check, including criminal history, certification registries, and employment history
  • Completion of training as required by Missouri Slope
