Job summary

AOM Healthcare is seeking a dedicated Activities Director for Concord Care Center of Toledo, a full-time position located at 3121 Glanzman Rd, Toledo, OH. The role focuses on developing and supervising engaging activities to enhance residents’ well-being and quality of life within a compassionate long-term care environment.

Ideal candidates will have two years of relevant experience, CNCA certification or equivalent, strong communication skills, and a valid driver’s license.

Qualifications

• Minimum two years in a social or recreational program in the past five years, including at least one full-time in a resident activities program in a healthcare setting.
• Certified by the National Certification Council for Activity Professionals; alternatives available as noted in item 2.
• Sufficient verbal and written communication skills to perform required tasks.
• Must possess a valid driver’s license with clean driving record.

Responsibilities

• Develops activities that provide sensory, social, and personal achievement opportunities for residents.
• Evaluates residents and creates individualized activity plans with data for medical records.
• Documents activity programs and completes care plans within seven working days of admission.
• Coordinates and supervises events, field trips, and resident council meetings; supports community involvement.
• Attends staff and committee meetings and participates in resident care planning conferences.
• Other duties as assigned.

AOM Healthcare is a premier long term care company, operating twenty one long term care facilities throughout the state of Ohio and is now hiring Full-Time Activities Director at Concord Care Center of Toledo located at 3121 Glanzman Rd, Toledo, OH.

Job Requirements & Qualifications:

• Develops activities providing the opportunity for residents to experience sensory input - touch, smell, taste, etc., group interaction and personal achievement.
• Evaluates each resident according to his/her background, interests, abilities, physical and mental limitations and needs. This shall serve as the base from which the individual activities program shall be developed. Provides accurate data as part of the MDS inter-disciplinary team.
• Documents/evaluates the individuals activity program using the appropriate assessment forms to be placed in the medical record, completing the activity care plan within seven working days of admission.
• Develops appropriate records, which indicate residence attendance and participation in the program with reference to resident response to the program.
• Develops a method to implement programs within a designated budget, allocated by the facility administrator.
• Attends and participates in staff meetings, department head meetings, designated committee meetings and resident care plan conferences. Also, shall attend mandatory staff meetings.
• Coordinates and Supervises special events, field trips and encourages residents to remain active in the community.
• Coordinates resident council meetings and ensures follow through on appropriate forms reviewed and signed by the facility administrator.
• Other duties as assigned.
• Two years of experience in a social or recreational program within the past five years, one of which was full-time in a resident activities program in a healthcare setting.
• Certified by the National Certification Council for Activity Professionals.
• In Lieu of the certification in item 2, may be an Occupational Therapist, Art Therapist, Music Therapist, Dance Therapist, Recreational Therapist or any Occupational Therapy Assistant.
• Sufficient verbal and written communication skills to perform the task required.
• Must have a valid driver’s license with clean driving record.
