Job summary

RiseMe is seeking an Activities Director to lead the development and coordination of resident activity services and programs. You will assess residents, create individualized recreation plans, and participate in interdisciplinary care planning to meet goals.

You should have state licensure as Activities Director (or related credentials), plus at least two years in social or recreational programming and one year in healthcare activities programs.

Qualifications

• Licensed or registered in state as Activities Director, Social Worker, Occupational Therapist (OT) or Occupational Therapy Assistant (OTA).
• Minimum two (2) years' experience in social or recreational program within the last five (5) years.
• Minimum one (1) year full-time experience in health care activities program.
• Certification as a Therapeutic Recreation Specialist preferred.

Responsibilities

• Interview and assess all residents prior to the initial Care Plan Conference; document this information in the medical record, develop an individual recreation plan based on the assessment and participate in Interdisciplinary Care Plan meetings
• Update assessments and plans as needed and required by state or federal regulations
• Develop monthly recreation program calendars that reflect and meet the needs of facility residents
• Communicate facility programs to residents, staff, family and volunteers
• Manage facility Volunteer Program
• Maintain departmental documentation that reflects services provided and resident progress towards goals
• In coordination with social services facilitate the residents in the organization and continued development of a Resident's Council
• Make job assignments and set priorities
• Serve as member of QAA committee

Education

Licensed or registered in state (Activities Director)

Therapeutic Recreation Specialist certification preferred

2+ years in social or recreational program

1 year full-time experience in health care activities program
