Description

1. Participate in planning and conducting individual, small, and large group activities.
2. Assist in providing effective communication between employees at all levels, residents, their families, support personnel, government agencies, and the public to meet the needs of residents, the community, and the facility.
3. Participate in community planning related to the interests of the facility and the needs of residents and their families.
4. Assist in developing the monthly activity calendar and maintaining attendance records.
5. Support the Quality Assurance and Assessment Committee in developing and implementing plans to address quality deficiencies as directed by the Activities Director.
6. Participate in discharge planning, and assist in developing and implementing activity care plans and resident assessments.
7. Assist in arranging transportation for residents, including outings, appointments, or discharges.
8. Develop a monthly activity schedule that includes outings, group activities, and in-room activities for residents who are bed-bound or isolated.
9. Encourage residents to participate in self-initiated activities such as hobbies, crafts, and reading; provide materials as needed, including Braille or audio books.
10. Assist with documentation, quarterly progress notes, and assessment records as assigned.
11. Maintain the cleanliness, orderliness, and security of the Activity Department.

Supervisory Requirements: This position has no supervisory responsibilities.

Qualifications:

• High school diploma or equivalent.
• Preferably one year of experience in a long-term care facility.

Skills:

• Ability to read and comprehend technical procedures, policies, and manuals.
• Effective communication skills to present information and respond to questions.
• Mathematical skills including understanding of fractions, percentages, ratios, and proportions.
• Problem-solving abilities and the ability to interpret various instructions.

Physical Demands: Standing, walking frequently; sitting occasionally; reaching, pushing, pulling frequently; talking and hearing frequently; tasting and smelling frequently; lifting up to 50 pounds regularly; occasional climbing, balancing, stooping, kneeling, crouching, or crawling. Visual acuity for close, distance, color, peripheral, and depth perception.
