Description

Shift Details: 9am to 5pm Mon-Fri and every other weekend is required.

About the Role: The Program and Outreach Assistant reports directly to the Program and Outreach Director. They assist in delivering resident-centered activities that promote wellness across emotional, physical, social, intellectual, and spiritual dimensions, aligning with Phoenix Senior Living's philosophy.

Responsibilities:

  • Refer to the resident profile and addendum for every new resident.
  • Facilitate personalized activities incorporating wellness dimensions, with resident care associate participation.
  • Recognize and respond to changes in resident condition.
  • Assist residents to and from activity rooms and life enrichment activities, especially in memory care units.
  • Maintain communication logs and activity areas.
  • Assist with the care and record-keeping of animals or plants involved in activities.
  • Manage inventory of activity supplies, games, and crafts.

Requirements:

  • High School Diploma or equivalent; experience in senior living preferred.
  • Ability to lead activities and motivate volunteers and staff.
  • Knowledge of assisted living and dementia care preferred.
