Job summary

Clarvida is seeking an ACT Nurse (RN) to join our Assertive Community Treatment (ACT) Team serving adults with severe and persistent mental illness. In this role, you will be a critical part of a multidisciplinary team providing intensive, community-based services designed to support recovery, stability, and independence.

ACT services are delivered where clients live, work, and spend time in the community. You will support a specialized caseload, helping clients achieve treatment plan goals

Qualifications

• Active RN in the State of Louisiana.
• Experience with individuals with severe mental illness and/or substance use disorders.
• Psychiatric nursing experience preferred.
• Valid Driver's License in good standing and current automobile insurance.

Responsibilities

• Provide nursing services as part of the ACT multidisciplinary team.
• Meet with clients in homes and community settings.
• Conduct basic health assessments, including vitals and labs.
