Job summary

Clarvida is seeking an ACT Nurse (RN) to join our Assertive Community Treatment Team serving adults with mental illness in the Mandeville/Greater New Orleans area. You will work within a multidisciplinary, community-based program delivering intensive services designed to support recovery, stability, and independence.

The role requires active Louisiana RN licensure, experience with severe mental illness, and a valid driver's license.

Qualifications

• Active RN license in the state of Louisiana.
• Experience with individuals who have severe and persistent mental illness and/or substance use disorders.
• Psychiatric nursing experience preferred.
• Valid driver’s license and current auto insurance.

Responsibilities

• Provide nursing services as part of the ACT multidisciplinary team.
• Meet with clients in homes and community settings.
• Conduct basic health assessments, including vitals and labs.
• Support medication management and long-acting injectable medications.
• Coordinate care with medical providers and psychiatric staff.
• Support clients with severe and persistent mental illness and/or substance use disorders.
• Participate in a rotating on-call schedule (≈1 week every 6-8 weeks).
• Provide client transportation using a company vehicle as needed.
• Maintain accurate, timely, and compliant documentation.
