RiseWell Community Services is seeking a Full-Time Team Leader to join our Assertive Community Treatment (ACT) Teams in New York. The Team Leader will supervise their ACT team while providing direct services on a multidisciplinary unit, ensuring compassionate communication and strong leadership within the clinical setting.

Qualified candidates bring a Master’s degree and relevant licensure; RN/NP or physician credentials are also acceptable.

Qualifications

• Master’s degree with social work/mental health license or nursing/physician licensure.
• Valid NYS driver’s license.
• Minimum 3 years of experience in a treatment or rehabilitation setting.

Responsibilities

• Supervise the ACT team and provide direct services on a multidisciplinary team.
• Lead and coordinate clinical services, ensuring collaboration and compassionate communication.
• Support treatment planning, compliance with regulations, and ongoing professional development.

RiseWell Community Services is looking to hire a Full-TimeTeam-Leader to join the Assertive Community Treatment (ACT) Teams.

In this role, the Team Leader will supervise their ACT team while providing direct services on a multi-disciplinary team. Our leaders serve as an essential component of this impactful, clinical team. Leadership skills, collaborative abilities, and compassionate communication are a must!

Qualifications

Master’s degree or higher with a license in social work or Mental Health Counseling, psychology, rehabilitation counseling (if rehabilitation counselor, C.R.C. preferred), OR licensure/registration as a registered nurse (RNs are encouraged to be master's level or nurse practitioner), or a physician. License required (LMSW, LMHC, LCSW, LMFT) . A valid and clean NY State driver's license. Minimum 3 years of experience in a treatment or rehabilitation setting.Fingerprinting, criminal record check, and approval from the NYS Office of Mental Health.
