The RN will collaborate with an ACT team to reduce inpatient hospitalizations, participate in daily huddles, and respond to crises as part of a 24/7 on-call rotation.

Qualifications

• Must be licensed as a Registered Nurse in Texas.
• Minimum of 1 year in Mental Health Nursing.
• Prior case management experience preferred.
• Two years of field-based nursing preferred.
• Knowledge of Trauma-Informed Care Practices required.
• Must have reliable transportation and a valid driver’s license.

Responsibilities

• Administers long-acting injectables to individuals served.
• Provides case management, skills training, medication training and support.
• Delivers psychosocial rehabilitation services with trauma-informed, person-centered care.
• Responds to on-call needs and after-hours crises.
• Supports access to medical, social, vocational, financial, educational services.
