Job summary

LifePath Systems in Texas seeks an ACT Registered Nurse to administer long-acting injectables and provide trauma-informed psychosocial rehab. The role emphasizes person-centered care and case management to help clients access medical, social, vocational, financial, and educational services in the community.

The RN will collaborate with an ACT team to reduce inpatient hospitalizations, participate in daily huddles, and respond to crises as part of a 24/7 on-call rotation.

Qualifications

  • Must be licensed as a Registered Nurse in Texas.
  • Minimum of 1 year in Mental Health Nursing.
  • Prior case management experience preferred.
  • Two years of field-based nursing preferred.
  • Knowledge of Trauma-Informed Care Practices required.
  • Must have reliable transportation and a valid driver’s license.

Job description

ACT RN will be responsible for administering long-acting injectables to individuals served is a central duty of this position. This position performs nursing and psychosocial rehabilitation services based in trauma informed and person driven care model to individuals in the most intensive level of care. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services. Provides person directed skills training, med training and support, and psychosocial rehab to develop coping and other skills needed to feel successful in their home and community. In addition, this position is periodically on call and will be expected to respond outside of regular business hours. The ACT team takes a collaborative approach and meets daily to staff individuals served in an effort to reduce inpatient psychiatric hospitalizations and foster independence. The ACT RN connects individuals served to primary physical health care and supports them as they apply for qualifying benefits. Promoting choice and autonomy through a strength-based model is necessary. Has full understanding of ACT fidelity model and meets requirements per model.

This position exemplifies service excellence by providing case management, skills training, medication training and support, crisis interventions and psychosocial rehabilitation services with a trauma-informed and person-centered approach. This case manager practices good stewardship of resources by ensuring all required trainings are completed prior to due date and center-issued equipment is kept in good repair. Provides person directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services.

Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, grants, SAMHSA ACT Fidelity and Lifepath Systems policies and procedures. This case manager builds positive relationships with community-facing agencies and always maintains a professional relationship with all community stakeholders and partners. Professional conduct with Peers, Leadership and Support teams is expected at all times. Promoting choice and autonomy through a strength-based model is necessary to assist individuals served with improving access to social determinants of health.

Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and community partners; to facilitate improvement in measurable ways, is central to LifePath Systems mission to serve. Ability to assess and de-escalate behavioral health crisis situations in calm and respectful manner.

Specific Targets:

Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record a minimum of 60 hours per month. 90% of service notes are signed within 2 business days of service. All required trainings are completed prior to due date. All audit scores exceed 80% quality standard. 90% of caseload has updated TRR and Financial Assessments. Will serve in on-call rotation to provide 24/7 crisis coverage for individuals on ACT team. Attendance at daily ACT huddles is mandatory.

If bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.

Qualifications
Education, Training, & Experience
  • Must be licensed as a Registered Nurse in the State of Texas.
  • Must have at least one year of Mental Health Nursing experience.
  • At least one year of Case Management experience preferred.
  • Two years of field-based nursing is preferred.
  • CCBHC knowledge and experience preferred.
  • Experience in Texas Recovery and Resiliency (TRR) preferred.
  • Knowledge of Trauma-Informed Care Practices required.
  • Must have reliable transportation and a valid driver’s license.
Knowledge, Skills, & Abilities
  • Experience providing psychosocial assessments, treatment planning, case management, service coordination, skills training, crisis intervention and symptom management services to individuals with serious mental illness or serious emotional disturbance.
  • Ability to assess individual needs, to coordinate services, and to communicate effectively.
  • Knowledge of the social services system and the ability to connect individuals with appropriate services.
  • Ability to write clearly and professionally on all services and internal communications.
  • Ability to think clearly and logically while dealing with potentially dangerous and/or emotional situations.
  • Ability to respond to individuals with patience, empathy and understanding.
  • Organized, detail-oriented, and computer-proficient.
  • Ability to demonstrate strong multi-cultural competency.
  • Bilingual (English/Spanish) preferred.
