RADIAS Health, a nonprofit providing mental health and substance use services in Minnesota, seeks a Mental Health Practitioner for its Assertive Community Treatment Teams. You will deliver trauma-informed, person-centered care in the community, develop treatment plans, and coordinate with a multidisciplinary team to support adults with mental illness and related conditions.

Qualifications

• Bachelor's or Master's Degree in Behavioral Sciences or related field.
• 2,000 hours supervised experience with Mental Illness.
• Strong teamwork and crisis intervention abilities.

Responsibilities

• Develop, write, implement and evaluate treatment goals and plans with clients and team.
• Develop comprehensive assessments and individualized treatment plans for clients.
• Assist with housing access and independent living skills, budgeting and daily tasks.
• Collaborate with vocational staff to promote competitive employment.
• Attend court hearings and complete required court reports.
• Prepare and file mandated reports and charting in a timely manner.
• Lead substance use treatment planning and group/individual sessions.
• Engage family members and landlords with client permissions as needed.
• Perform shift management and provide on-call crisis intervention.
• Attend all scheduled team meetings and participate in ongoing professional growth.
