Job summary

Hope Network is seeking an ACT QMHP to join a multidisciplinary team providing person-centered mental health services to individuals with significant needs. The role involves assessments, treatment planning, crisis response, and coordinating care to support recovery and independence.

The position emphasizes trauma-informed, recovery-focused care within a team-based model, with on-call rotation and collaboration with families and community resources.

Qualifications

• Bachelor's degree in a related human services field or QMHP credential.
• At least one year of experience in a clinical setting serving individuals with significant mental health needs.
• Driver's license with <5 points and willingness to transport persons served.

Responsibilities

• Build positive, supportive relationships with individuals served by meeting with them regularly in their homes and communities.
• Promote recovery, independence, safety, and community integration.
• Complete assessments, identify strengths and needs, and develop person-centered treatment plans with individuals and the ACT team.
• Provide education and skill-building related to mental health, daily living, coping skills, and symptom management.
• Coordinate care and connect individuals with healthcare providers, families, and community resources to support housing, employment, education, benefits, transportation, and daily living.
• Support medication management and other health-related needs as appropriate.
• Provide crisis intervention and participate in the ACT team's on-call rotation.
• Participate in team meetings, treatment planning, and collaborative decision-making.
