Job summary

People Encouraging People in Glen Burnie, MD is seeking a full-time ACT Case Manager to plan and implement rehabilitation services for clients with co-occurring mental illness and substance use disorders. The role includes acting as a resource to families and staff and coordinating treatment plans with the team.

The position requires a Bachelor’s in a mental health field (or equivalent experience), valid driver’s license, and ability to work independently with supervision as needed.

Qualifications

• Bachelor’s Degree in mental health field preferred or three years equivalent experience.
• Two to three months OJT required for efficient job performance.
• Must have a valid driver's license and a clean driving record.

Responsibilities

• Design and implement program elements to meet client needs in mental health, harm reduction, prevention, and recovery.
• Organize structured group and individual activities aligning with client goals for mental health.
• Provide consultation and materials to staff through clinical team meetings and conferences.
• Complete documentation including service, billing data, and clinical records.
• Support Individual Treatment Planning with client goal formulation, monitoring, and progress reporting.
• Perform direct service interventions as indicated in the ITP, in coordination with the treatment team.
• Assist in developing annual service goals and measuring/reporting progress.
• Identify resource needs and procure materials per agency procedures.
• Establish and maintain communication with other providers and leaders to ensure resource availability.
• Provide case management for an assigned group of clients and coordinate the treatment team activities.
• Conduct comprehensive assessment of all domains of a person’s life.
• Provide on-call crisis intervention during nighttime and weekend hours.
• Become an expert of the ACT philosophy and maintain high fidelity to the ACT Model of Care.
• Complete the process of getting clients qualifying entitlements in a timely manner.
