Job summary

People-Encouraging-People in Glen Burnie, MD seeks an ACT Case Manager to plan and implement rehabilitation services for clients with co-occurring mental illness and substance abuse. You will act as facilitator, provide case management and ensure thorough documentation as part of a treatment team.

The role requires a Bachelor’s degree in mental health and a valid driver’s license, plus 2–3 months of on-the-job training.

Qualifications

• Bachelor’s degree in mental health field preferred and/or three years equivalent work experience in mental health.
• Two to three months on-the-job training required for efficient job performance.

Responsibilities

• Plan and implement rehabilitation program services for clients with co-occurring mental illness and substance abuse.
• Act as facilitator and resource to other staff in families education, illness management and crisis intervention.
• Design and implement program elements for mental health, harm reduction, prevention, recovery and related areas.
• Organize structured group and individual activities aligned with client goals.
• Provide consultation and materials through clinical team meetings and conferences.
• Complete required documentation including service and billing data and clinical records.
• Participate in the Individual Treatment Planning process by assisting with goal formulation and progress reporting.
• Provide case management for an assigned group of clients and coordinate treatment team activities.
• On-call crisis intervention during nighttime and weekend hours.
• Maintain high fidelity to the ACT Model of Care.
• Assist in getting clients entitlements in a timely manner.
