Job summary

Community Care Alliance in Woonsocket, RI is seeking a Case Manager for the Assertive Community Treatment Team to support individuals with serious mental illness and co-occurring substance use in their homes.

The role emphasizes ongoing assessments, crisis intervention, treatment planning with clients, and collaboration with community resources to promote recovery and reduce crises. This is a full-time position with benefits.

Qualifications

• BA/BSW in a human services related field is required.
• Knowledge of substance abuse and mental health is a plus.
• Experience with Electronic Health Record systems is preferred.
• Approved driver's record and reliable transportation required.

Responsibilities

• Assess and evaluate clients on an ongoing basis to provide support.
• Crisis intervention when needed to prevent hospitalizations.
• Develop and implement treatment goals with clients to promote recovery.
• Collaborate with community resources to support recovery and mental health.
• Provide comprehensive community support to individuals living in their homes.
