Job summary

Spurwink in Maine is seeking an Assertive Community Treatment (ACT) case manager to join an interdisciplinary team serving adults with serious mental illness.

The role requires MHRT-C certification or eligibility, or related credentials, and a Bachelor’s degree. A valid driver’s license and willingness to conduct home visits and after-hours outreach are essential. The position offers a competitive hourly wage with a comprehensive benefits package.

Qualifications

• MHRT-C certification or eligible, or a Certified Rehabilitation Counselor or Employment Specialist, or a Certified or Licensed Alcohol and Drug Counselor.
• Bachelor's degree from an accredited four-year institution preferred.
• Must maintain a current driver’s license and clean driving record.

Responsibilities

• Provide intake/assessment when clients are referred and ongoing assessment.
• Facilitate ISP team meetings and develop a plan of care with crisis planning.
• Oversee ISP implementation and monitor progress.
• Maintain contact with team members and other professionals; collaborate with Psychiatry, Nursing, SUD counselor, Vocational Rehab, and peers.
• Identify and assess health-related risk factors and help clients access resources.
• Advocate for clients and families.
• Provide home visits and after-hours outreach as needed.
• Partner with primary care and other providers.
• Participate in supervision and required trainings.

Job description

Come join a vibrant, interdisciplinary team serving the complex needs of adults with serious mental illness in the Lakes Region. We are looking for a collaborative, engaging, and driven Assertive Community Treatment (ACT) case manager who has their MHRT-C or is MHRT-C eligible. A Certified Rehabilitation Counselor or Employment Specialist, or Certified or Licensed Alcohol and Drug Counselor will be considered with relevant experience. In this position you will provide community-based case management as part of ACT's team, working with individuals living with Severe and Persistent Mental Illness.

This position pays $23.00-$27.00 per hour depending on experience.

DUTIES:

• Provide intake/assessment when clients are referred, and ongoing assessment as required.
• Facilitate Individual Service Plan team meetings and develop a plan of care (ISP) for the adult with input from the team. Plan will include a crisis plan.
• Oversee implementation of ISP, monitor and document progress.
• Maintain regular contact with team members and other community professionals involved with client. Participate in a team-based approach to care, by working in collaboration with Psychiatry provider, Clinical Team Leader, Nurse, Substance Use Disorder Counselor, Case Manager, Vocational Rehabilitation Provider, and Peer Partner.
• Identify and assess physical and behavioral health related risk factors and ensure that clients and families are aware of all community services and resources. Assist clients and families in accessing resources.
• Advocate for clients and families.
• Provide support through phone contact and face-to-face meetings through home visits and community outreach. Home visiting required, as well as ability to work outside traditional work hours to meet clients scheduling needs.
• Partner with clients' primary care, medical, and other providers.
• Participate in individual and group supervision, required training, and case consultations.

MHRT-C certification or MHRT-C eligible, or a Certified Rehabilitation Counselor or Employment Specialist, or a Certified or Licensed Alcohol and Drug Counselor. Bachelor's degree from an accredited four (4) year institution of higher learning preferred. Must maintain a current driver's license and a clean driving record as outlined in Spurwink's Driving Policy.
