Spurwink in Lewiston, ME, is seeking a collaborative ACT case manager to serve adults with serious mental illness. You will provide community-based case management as part of an interdisciplinary ACT team, handling intake, ISP planning, and progress monitoring.

The role involves home visits, after-hours scheduling flexibility, and coordination with psychiatry, nursing, and community partners to connect clients with needed services.

Qualifications

• MHRT-C certification or eligibility, or related counseling credentials.
• Bachelor's degree from an accredited four-year institution.
• Valid driver's license with clean driving record per Spurwink policy.

Responsibilities

• Facilitate ISP team meetings and develop a plan of care with input from the team.
• Oversee ISP implementation, monitor and document progress.
• Maintain regular contact with team members and other professionals involved with client.
• Identify and assess health-related risks and connect clients to services.
• Advocate for clients and families.
• Provide home visits and community outreach; flexible hours as needed.
• Partner with clients' primary care and other providers.
• Participate in supervision, training, and case consultations.

Job description

Come join a vibrant, interdisciplinary team serving the complex needs of adults with serious mental illness in Lewiston. We are looking for a collaborative, engaging, and driven Assertive Community Treatment (ACT) case manager who has their MHRT-C or is MHRT-C eligible. A Certified Rehabilitation Counselor or Employment Specialist, or Certified or Licensed Alcohol and Drug Counselor will be considered with relevant experience. In this position you will provide community-based case management as part of ACT's team, working with individuals living with Severe and Persistent Mental Illness.

This position pays $23.00-$27.00 per hour depending on experience.

DUTIES:

• Provide intake/assessment when clients are referred, and ongoing assessment as required.
• Facilitate Individual Service Plan team meetings and develop a plan of care (ISP) for the adult with input from the team. Plan will include a crisis plan.
• Oversee implementation of ISP, monitor and document progress.
• Maintain regular contact with team members and other community professionals involved with client. Participate in a team-based approach to care, by working in collaboration with Psychiatry provider, Clinical Team Leader, Nurse, Substance Use Disorder Counselor, Case Manager, Vocational Rehabilitation Provider, and Peer Partner.
• Identify and assess physical and behavioral health related risk factors and ensure that clients and families are aware of all community services and resources. Assist clients and families in accessing resources.
• Advocate for clients and families.
• Provide support through phone contact and face-to-face meetings through home visits and community outreach. Home visiting required, as well as ability to work outside traditional work hours to meet clients scheduling needs.
• Partner with clients' primary care, medical, and other providers.
• Participate in individual and group supervision, required training, and case consultations.

QUALIFICATIONS:

MHRT-C certification or MHRT-C eligible, or a Certified Rehabilitation Counselor or Employment Specialist, or a Certified or Licensed Alcohol and Drug Counselor. Bachelor's degree from an accredited four (4) year institution of higher learning preferred. Maintain a current driver's license and a clean driving record as outlined in Spurwink's Driving Policy.
