Job summary

PRAIRIE VIEW INC in Newton, KS seeks an Attendant Care Worker to join the ACT team. You will deliver community-based, person-centered support to adults with serious mental illness, helping with daily living, health management, and community integration.

You will work with an interdisciplinary team, maintain documentation within 24 hours, and advocate for participants' rights while coordinating care plans and resources in local settings.

Qualifications

• High school diploma or equivalent; certification eligibility to provide Peer Support Worker service in Kansas.
• Must be 18+ years old and able to pass background checks (KS KBI/Registry).
• Valid Kansas driver’s license and be insurable; able to provide transportation as needed.

Responsibilities

• Assist participants with daily living skills (meals, housekeeping, hygiene, laundry, self-management).
• Provide in-home and community support per participant care plan.
• Support health management, health literacy, and self-direction.
• Attend ACT team meetings and contribute to clinical supervision; advocate for participants’ needs.
• Provide crisis intervention and connect to community resources; maintain timely documentation.
