Are you looking for a hands-on, rewarding role where you can directly help individuals in your local community maintain their independence and well-being? We are seeking a self-motivated Attendant Care Worker to join our Assertive Community Treatment (ACT) team in Newton, KS.

In this role, you will work directly with adults experiencing serious mental illness and co-occurring health conditions, helping them build daily living skills, access community resources, and live successfully in their homes.

Position Overview

As part of an interdisciplinary mental health team, you will deliver community-based, person-centered support designed to reduce hospital stays and emergency room visits. You will manage your own daily schedule while visiting participants in their homes and local community settings, guiding them through independent living activities, physical health management, and community integration.

Key Responsibilities

• Daily Living & Skill Building: Assist participants in learning, practicing, and maintaining essential daily living skills such as meal preparation, basic housekeeping, laundry, personal hygiene, and self-management.
• In-Home & Community Support: Provide self-directed, person-centered services aligned with each participant's care coordinator plan to support independent living in their home environment.
• Health & Wellness Engagement: Help participants understand and manage their physical health conditions, encouraging self-direction, health literacy, and motivation.
• Team Collaboration & Advocacy: Attend daily ACT team meetings to coordinate care, participate in clinical supervision, and advocate for participants' needs and rights.
• Crisis Intervention & Resource Connection: Support participants during moments of crisis, link them to necessary community services, and encourage social engagement.
• Documentation: Maintain accurate, timely clinical records (progress notes completed within 24 hours, utilizing efficiency tools like Bells A.I.).

Qualifications

POSITION QUALIFICATIONS:

• Minimum Education: High school diploma or equivalent; certification, or ability to become certified upon being hired, in the State of Kansas to provide the Peer Support Worker service, which includes criminal, abuse/neglect registry and professional background checks, and completion of a state approved standardized basic training program
• Minimum Experience: Provider must self identify as a present or former primary client of mental health services
• Must be at least 18 years of age
• Must have excellent communication skills – listening, verbal, and written
• Must be able and willing to be flexible, a self starter, punctual, and have the ability to maintain a schedule
• Must have a valid Kansas driver’s license, be insurable through Prairie View insurance company, and be willing to provide transportation to patients as needed
• Must have ability to maintain professional boundaries with peers
• Must be proficient at basic keyboarding/word processing skills
• Must be able to pass Kansas Bureau of Investigation Screen and the Central Registry Screen for both adults and children
