Job summary

Concierge Home Care is hiring a Home Health Account Executive based in the South Jacksonville, FL area. The role focuses on building referral relationships, advocating for patients, and coordinating with clinical teams to ensure smooth transitions from facility to home care.

Candidates should have healthcare sales experience, excellent communication skills, and the ability to travel within the assigned territory. We offer a supportive team, growth opportunities, and a flexible work environment.

Qualifications

• Previous healthcare sales experience required (SNFs, DME, ortho, cardio etc.)
• Excellent interpersonal communication and presentation skills
• Detail-oriented and reliable travel within territory

Responsibilities

• Build Relationships: Develop and maintain strong connections with referral sources to meet or exceed sales goals.
• Patient Advocacy: Ensure safe, seamless transitions from healthcare settings to home care.
• Collaborate with Teams: Participate in weekly case conferences and coordinate with community healthcare teams.
• Community Education: Educate residents, caregivers, and wellness directors.

• Admission Coordination: Oversee referral-to-admission processes and ensure required documentation.
• Executive Communication: Maintain regular contact with the community’s executive team.

Your Role as a Home Health Account Executive

• Build Relationships: Develop and maintain strong connections with referral sources to consistently meet or exceed sales goals.
• Patient Advocacy: Act as a patient advocate, ensuring a safe and seamless transition from healthcare settings back to their home within the community.
• Collaborate with Teams: Participate in weekly case conference meetings and coordinate with community healthcare teams.
• Community Education: Provide ongoing education to residents, caregivers, and wellness directors.
• Continuity of Care: Ensure smooth communication and continuity between physicians, community, and home care teams.
• Admission Coordination: Oversee the referral-to-admission process, ensuring all required documentation is obtained and processed efficiently.
• Executive Communication: Maintain regular communication with the community’s executive team to ensure seamless resident care and continuity.

Qualifications

• Clinical background (preferred)
• Previous healthcare sales experience required, such selling in skilled nursing facilities (SNFs), DME, ortho, cardio, etc., OR previous experience as a clinical liaison
• Excellent interpersonal communication and presentation skills (required)
• Proficiency in Microsoft Office (preferred)
• Detail-oriented (required)
• Ability to travel within the assigned territory and to sales meetings as required
• Exceptional customer service and communication skills (both verbal and written)
• Valid driver’s license, auto insurance, and reliable transportation (required)
