Job summary

Oakland Community Health Network in Pontiac, MI seeks an Access Information and Referral Specialist to provide information and referrals for mental health, substance use disorders, and developmental disabilities. Responsibilities include greeting visitors, managing referrals, coordinating communications, and ensuring short wait times.

Qualifications

  • High school diploma or GED required; two years of related coursework preferred.
  • Minimum two years of experience in human services, healthcare access, or related fields.
  • Bilingual Spanish desirable; Bachelor’s in human services preferred.

Responsibilities

  • Greet visitors at RCC in a warm, trauma-informed manner.
  • Manage referrals via walk-ins, email, phones, and faxes; coordinate to clinicians.
  • Serve as primary contact for hospitals, jails, courts, and referral sources.
  • Coordinate communications for referrals, admissions, discharges, and appointments.
  • Provide information and advocacy on the Access process.
  • Complete documentation and warm transfers to clinicians.
