Job summary

Lutheran Community Services NW (LCSNW) seeks an Intake/Access Coordinator to manage intake and assessment for multicultural counseling, family and community support services.

You will gather essential information, conduct screenings, and ensure smooth transitions into appropriate programs while maintaining confidentiality and HIPAA compliance.

Qualifications

  • Excellent organizational and interpersonal skills required.
  • First point of contact for clients; gather information and screen.
  • Ability to obtain QMHA within 30 days is required.

Responsibilities

  • Serve as the first point of contact for clients via phone, email, referrals, or in person.
  • Conduct initial mental health and substance use screenings.
  • Triage to determine appropriate services or level of care.
  • Track and monitor student grades and attendance through families and school staff.
  • Coordinate intake appointments and enrollment into behavioral health programs.
  • Collaborate with clinicians and other professionals for accurate assessment and referrals.
  • Maintain accurate client records in the EHR system.
  • Provide appointment reminders and necessary documentation to clients.
  • Generate reports on intake and referral activities.

HOW YOU WILL MAKE AN IMPACT:

The Intake/Access Coordinator plays a crucial role in facilitating the intake and assessment process for individuals seeking multicultural counseling,familyand community support services. This position requires excellent organizational and interpersonal skills to effectively coordinate and manage the intake procedures. The Intake Coordinator serves as theinitial point of contact for clients, gathering essential information, conducting screenings, and ensuring a smooth transition into theappropriate programs or services. AddictionsPSS, CADC, or CRM, andQMHAorQMHA-Rwith theability to obtainQMHAwithin30 daysis required.

  • Serve as the first point of contact for individuals seeking services, either through phone calls, emails, referrals, or in-person interactions.
  • Conduct initial mental healthand substance use screenings to gather necessary information about the client's needs, history, and current mental health status.
  • Triage based on the client's situation to determine appropriate servicesor level of care.
  • Track andmonitorstudent grades and attendance through regularcontactwith families and teachers/school counselors.
  • Collaborate with clinicians, chemical dependency counselors, and other professionals to ensure accurate assessment and placement of clients.
  • Schedule intakeappointments for clients and coordinate their enrollment into behavioral health programs or services.
  • Collaborate with internal staff and external providers to coordinate referrals and ensure timelyaccess to appropriate services.
  • Maintainaccurateand up-to-date records of client information, including demographic data, assessment results, and referral details.
  • Communicate with clientsregarding appointment reminders, requireddocumentation, and any necessary follow-up information.
  • Prepare andmaintain intakeforms, assessment tools, and other necessary documentation for client records and program compliance.
  • Enter client information accurately into the electronic health records (EHR) system or any other designated database
  • Generate reports and statistical data related to intake and referral activities, as required by the organization or regulatory agencies.
  • Maintain confidentiality and adhere to all relevant privacy regulations, such as HIPAA (Health Insurance Portability and Accountability Act)and 42 CFR Part 2.
  • Communicate effectively with clients, families, and referring agencies to address inquiries, provide information, and ensure a positive experience.
  • Collaborate with the clinical team to discuss assessment findings, treatment options, and appropriate referrals.
  • Stay informed about available behavioral health resources, treatment options, and community programs to provide accurateinformation and referrals.
  • Develop andmaintain relationships with community organizations, healthcare providers, and other stakeholders to facilitateseamless referrals and collaborations.
  • Maintain compliance with all relevant program/department requirements, including and not limited to adhering to all record-keeping and reporting requirements.
  • Model and encourage direct, professional communication among staff members and facilitate development of a positive staff support network.
  • Demonstrate awareness and sensitivity in the delivery of client services as it relates to the cultural and socioeconomic characteristics of the populations served.
  • Other duties as assigned.
HOW YOU WILL BE A GREAT FIT:

  • Excellent communication skills with the ability to read, write, and speak in English. Fluency in theadditional following language(s): Arabic, Burmese, Dari, French, Karen, Pashto, Rohingya, Spanish, Somali, Swahili, and Ukrainian preferred.
  • Standard LCSNW criminal clearance, including national sex offender, ORCHARDS, SAM/OIG.
