Qualifications

• Board certification or eligibility in Internal Medicine or Family Medicine.
• Fellowship in Geriatrics is a plus but not required.
• Licensed to practice in New Hampshire and Vermont.

Responsibilities

• Provide post-acute and long-term care at partner facilities with a full continuum of care.
• Offer outpatient geriatrics consultations in the clinic.
• Provide inpatient geriatric consults and home-based primary care through the Dartmouth Health at Home program.
