Job summary

Dartmouth Health is seeking an experienced Advanced Practice Provider (APN/PA) to join the Geriatric Medicine team at Dartmouth Hitchcock Medical Center in Lebanon, NH. The role emphasizes primary care for older adults within an interdisciplinary team and offers support such as care coordination, dietitian consults, pharmacy services, and embedded psychiatry services.

Applicants should have prior advanced practice experience in primary care and preferably geriatrics, with NH APRN or PA licensure

Geriatric Medicine Advanced Practice Provider

Dartmouth-Hitchcock Medical Center- Lebanon, NH

Dartmouth Health is seeking an experienced Nurse Practitioner or Physician Assistant to join our Geriatric Medicine team to provide primary care services in Lebanon, New Hampshire. The newly formed Section of Geriatric Medicine formally nested under Internal Medicine, led by Section Chief, Dr. Serena Chao, and VP of the Geriatric Centers of Excellence, Dr. Ellen Flaherty, is an interdisciplinary team with support for providers including: care coordination, dietitian consults, pharmacy services, and embedded psychiatry services.

Your involvement in working within a team is an important aspect of this position but also with the ability to work autonomously.

Prior advanced practice experience is desired which should include primary care and preferably geriatrics settings as well.

Dartmouth Hitchcock Medical Center (DHMC) and Dartmouth Hitchcock Clinics Services for Older Adults

• The Aging Resource Center provides older adults, families, and community members with information, education, and support to help them live healthier and more informed lives.
• The Geriatric Emergency Department (GED) provides emergency and trauma care for older patients. DHMC’s Emergency Department has earned Level 1 accreditation from the American College of Emergency Physicians, making the medical centre the only rural academic centre in the United States to earn this distinction.
• Geriatric Psychiatry provides mental health support for older adults. DHMC geriatric psychiatrists conduct comprehensive outpatient evaluations and offer inpatient consultations and a cognitive evaluation clinic. These services are designed to identify psychiatric illnesses across the spectrum, from mild to severe.
• Honoring Care Decisions promotes and improves advanced care planning at DHMC, Dartmouth Hitchcock Clinics, and in our communities. We support individuals and families to have meaningful conversations about what matters most in life at a time when patients are unable to speak for themselves.
• Primary Care offers accessible, comprehensive, and compassionate care for individuals and their families, with an emphasis on the whole person and the role of the family in health and illness. Our Family Medicine and Internal Medicine teams include providers who focus on the care of older adults.
• The Geriatric Surgery Program offers care and support for older adults and their families who need surgery. Our team includes experts in surgery, anaesthesiology, nursing, psychiatry, geriatrics, and care management. Before surgery, we identify patients who may be at a higher risk of delirium, the most common issue older adults face after surgery. We work closely with these patients to help improve their health before surgery and to ensure they have support after discharge from the hospital. While patients recover in the hospital after surgery, we monitor their progress and offer extra care as needed. Our approach to recovery is holistic. We work closely with services such as Healing Arts and Spiritual Health.
