Qualifications

• High School Diploma or GED is required.
• Three years of experience in Family Health Advocate, Eligibility Assistant, Outreach Worker, or Certified Application Counselor roles.
• Fingerprint Clearance Card: Level I, current or applied within seven working days.
• Preferred: Bilingual English/Spanish.

Responsibilities

• Determine eligibility for services and assist with ACA Marketplace enrollment.
• Provide training and information on CMS programs and AHCCCS.
• Generate reports and perform administrative/documentation tasks for program systems.
• Maintain confidentiality per HIPAA and organizational policies.
• Represent Elk Ridge Community Health in community events and outreach activities.
