Job summary

City of New York's NYC Health Department invites applicants for the Community Associate role within DFCH. The position supports maternal, infant and reproductive health information delivery, intake screening, and referrals to abortion services.

You will perform needs assessments, maintain privacy, and document interactions in the data system. Candidates should have a high school diploma with experience in community work or related fields, strong communication skills, and a commitment to health

Qualifications

• High school graduation or equivalent with three years of community work or related duties.
• Education and/or experience equivalent to the above.

Responsibilities

• Answer calls to provide information or referral to abortion services and conduct needs assessments.
• Provide empathetic, professional communication.
• Connect callers to care providers, navigators, and financial assistance.
• Record information into the data system and follow scripts.
• Maintain daily contact with supervisor and ensure privacy.
• Obtain consent for warm handoffs.
• Protect confidentiality of individuals.
