Puget Sound Home Health & Hospice in Tumwater, WA is seeking an experienced Hospice RN After Hours RN to provide high‑quality after‑hours hospice nursing care across Thurston, Grays Harbor, and Mason counties. This full‑time role follows a 7 on/7 off schedule with a 5:00 PM–Friday 8:00 AM and Friday 5 PM–Monday 8 AM shift pattern.

You must hold a WA RN license, have hospice experience, and current BLS. A reliable vehicle and valid license are required.

Qualifications

• Current Washington State Registered Nurse (RN) license.
• Hospice agency experience required.
• Current AHA BLS certification.
• Strong clinical assessment and independent decision‑making skills.
• Ability to work autonomously during evening, night, and early‑morning hours.
• Reliable, insured vehicle and valid driver’s license.

Responsibilities

• Deliver after‑hours hospice nursing visits, urgent care interventions, and skilled assessments
• Complete in‑person visits as required and assigned by Triage RN
• Provide pain and symptom management in accordance with hospice standards of care
• Educate patients and families on hospice philosophy, medications, and care expectations
• Document all assessments and interventions accurately and efficiently
• Communicate promptly with the interdisciplinary hospice team (IDT) to ensure continuity of care
• Participate in QAPI and quality improvement initiatives
