Job summary

Smith County Health and Rehab is seeking a Licensed Practical Nurse (LPN) to join our resident-focused team. We offer day and night shifts, full-time and part-time roles, with competitive pay and comprehensive benefits.

The LPN will provide bedside care, monitor vital signs, administer medications, and support residents and families in a compassionate environment. CPR and current state licensure are required, with experience in long-term care preferred.

Qualifications

• Current LPN license in the state.
• Up to date CPR certification.
• At least one year of LPN experience in a Long Term Care setting preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Provide bedside care including personal hygiene maintenance.
• Record vital signs and monitor patient status.
• Administer oral medications and injections.
• Relay information between patients, RNs, and physicians.
• Perform wound care and dressing changes.
• Maintain accurate medical records and charting.

Licensed Practical Nurse (LPN) Job Description:

• Provide bedside care to patients including personal hygiene maintenance
• Record the patients' vital signs (including blood pressure, pulse, and respiratory rate)
• Collect patient's health information
• Relay information between patients, registered nurses, and physicians
• Give oral medications and injections to patients
• Test a patient's blood sugar levels
• Urinary catheter insertion and care
• Wound care and dressing change
• Assist patients with personal hygiene
• Monitor for changes in patients
• Proper charting, documentation, and maintenance of patient's medical records
• Asks questions when in doubt for clarification by the Registered Nurse.
• Acts as a role model for ancillary personnel.
• Maintains the confidentiality of the patient's clinical records.
• Exceptional staff training and customer service skills
• Compassionate and friendly demeanor

Licensed Practical Nurse (LPN) Requirements:

• Current LPN license in the state.
• Up to date first aid and CPR certification.
• At least one year of LPN experience in a Long Term Care Setting preferred.
• Knowledge of MatrixCare preferred.
