Hope Network / Bay Valley House is seeking a Direct Support Professional for the 3rd shift (full-time, 32 hours) at Bay City, MI. You will support individuals to build independence, assist with daily living activities, and help them engage in the community with empathy and reliable care.

The role emphasizes safety, communication, and teamwork, with opportunities to participate in social events and community integration while maintaining accurate documentation and collaboration with care teams.

Qualifications

• High school diploma or GED is required.
• Valid State of Michigan driver's license is required.
• Ability to lift up to 50 lbs and perform basic care tasks.
• Ability to become CPR/First Aid certified.
• Shift availability including weekends and holidays.
• Basic computer skills and reliable attendance.

Responsibilities

• Monitor safety 24/7 with constant attention and checks.
• Support crisis prevention, intervention, and resolution.
• Build trust through consistent, respectful communication.
• Assist individuals to participate in community activities and social events.
• Provide assistance with activities of daily living as needed.
• Attend medical appointments and relay information accurately.
• Document care plans clearly and timely.
