Job summary

PharMerica is seeking a full-time Pharmacist for Wilkes-Barre, PA, 2nd shift. This non-retail, closed-door role serves long-term care and senior living clients, with a Monday–Friday 3pm–11pm schedule and no weekends.

You will review medications, collaborate with physicians, maintain patient profiles, ensure safety and regulatory compliance, and support clinical teams. Strong clinical judgment and licensure in the state are required.

Qualifications

• Bachelor’s Degree in Pharmacy; PharmD preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of CMS, DEA, FDA guidelines.
• Proficiency in EMRs and pharmacy information systems.
• Strong clinical judgement and effective communication.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, interactions, and route of administration.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes.
• Maintain up-to-date patient/resident profiles and medical records.
• Ensure monitoring laboratory tests for high-risk medications are ordered and reviewed.
• Collaborate with physicians, nurses, and healthcare professionals for clinical support.
• Document communications and interventions in the medical record.
• Enforce safe handling, dispensing, and management of medications.
• Ensure compliance with state and federal pharmacy regulations.
• Report medication errors to Pharmacy Manager as required.
• Educate clinical team on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
