Grace Management, Inc. is seeking a Licensed Practical Nurse (LPN) for the 2nd shift in Chicago, IL.

The part-time, on-site role supports a compassionate senior living community with responsibilities for resident care leadership and medication administration under physician orders. Qualifications

• Graduate of accredited school of nursing.
• Must be a licensed nurse in state where you work.
• Minimum of two years nursing experience.
• Ability to read, write, and speak in English.
• Experience in an Assisted Living setting preferred.
• Be in good physical health and free of communicable diseases; lift 40 pounds.

Responsibilities

• Plans and delegates work assignments ensuring effective shift coverage; coordinates care on the shift.
• Participates in and supervises the delivery of personal care to residents.
• Implements the service plans and collects data regarding residents' health status.
• Acts as team leader for shift; coaches RCAs/MTs on care practices and standards; writes disciplinary reports as needed.
• Maintains professional demeanor with residents and families.
• Reports suspected abuse and takes appropriate action during the shift.
