Job description

BAYADA Home Health Care is currently seeking full-time, part-time, and per diem Physical Therapists to join our team at our Harrisburg Visits Office!

This office services adult and geriatric clients on a per-visit basis in Perry County.

Compensation: $20,000 Sign-on Bonus for Full-Time! This amount will be paid out throughout your first year of employment.

As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to clients, enabling them to remain at home with appropriate medical support.

Responsibilities include:

1. Making home visits to clients in designated areas.
2. Performing diagnostic tests and measurements, including mobility, joint range, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
3. Developing and implementing individualized care plans with manual therapeutic exercises, gait training, balance, and other interventions.
4. Assessing and revising care plans regularly, collaborating with nursing staff as needed.
5. Educating clients and their families on rehabilitative care and activities to promote health, safety, and independence.
6. Documenting observations, interventions, and evaluations accurately using electronic medical records on a touch pad tablet.

Qualifications:

1. Current Pennsylvania Physical Therapist (PT) license.
2. Graduation from an American Physical Therapy Association-approved program.
3. Ability to work independently and manage time effectively.
4. Strong interpersonal, organizational, and problem-solving skills.
5. Proficiency with computers; experience with electronic medical records preferred.
