Job summary

Lexington Premier Nursing & Rehab is seeking a compassionate RN to join our skilled nursing facility team. You will provide direct patient care, administer medications, and coordinate with physicians and therapists to ensure optimal resident outcomes.

Requirements include a KY RN license and ADN or BSN; BLS is required, ACLS preferred. This role involves mentoring staff and participating in on-call rotations, with a focus on high-quality, compliant care.

Qualifications

• Education: ADN or BSN from an accredited program.
• Licensure: current KY RN license.
• Experience: 1 year in skilled nursing or LTC preferred but not required.
• Certifications: BLS required; ACLS preferred.

Responsibilities

• Provide direct patient care including assessments and medication administration.
• Coordinate with physicians, therapists, and staff to develop care plans.
• Communicate effectively with residents, families and the healthcare team.
• Maintain accurate patient records in accordance with policies and standards.
• Mentor and supervise LPNs and CNAs, ensuring adherence to care protocols.
• Ensure compliance with state and federal regulations, including infection control.
• Respond promptly to medical emergencies using clinical judgment.
• Participate in weekend, night, and holiday on-call rotation.
