Job summary

West Texas Centers is seeking caregivers for ongoing Case Management. Starting pay is $23.34 per hour with a career ladder and incentives.

The role involves helping individuals access medical, social, vocational, and educational services, coordinating care, monitoring progress, advocating, resolving crises, and engaging clients in activities to reduce symptoms and improve community functioning. Applicants must reside within 70 miles of the duty site, be able to travel using a personal vehicle,

Qualifications

• Bachelor's or advanced degree from an accredited college or university.
• Must have and maintain a Valid Drivers License and driving record acceptable for insurability.
• Must pass background check and drug screening.
• Applicants must reside within 70 mile radius of duty site

West Texas Centers is searching for highly motivated individuals to perform on-going Case Management to people served. Starting Pay:$23.34

Job Functions include assisting individuals in gaining needed services:

• access to medical, social, vocational, educational
• coordination of services
• service monitoring
• advocacy
• crisis resolution
• engaging clients in activities designed to reduce the symptoms of mental illness and to increase the ability of the individual to function in the community.

Requires flexible hours, involves extensive paperwork, and will require self-directed performance. Travel and the use of a personal vehicle is required.

Position Requirements

• Required: Bachelor's or advanced degree from an accredited college or university
