Cityscape Healthcare is seeking experienced Nurse Practitioners (NPs) or Physician Assistants (PAs) to join our growing mobile care network as independent contractors (1099). This role offers the opportunity to provide in‑home and in‑clinic care, perform health assessments, manage chronic and acute conditions, and deliver advanced wound care. Providers operate with a high level of autonomy while collaborating with a supportive clinical team.
Deliver advanced wound care services, including assessment, debridement, and development of individualized treatment plans.
Provide mobile care in patient homes, assisted living facilities, and partner locations.
Diagnose and manage chronic and acute conditions using a comprehensive, whole‑patient approach.
Document all patient encounters, treatments, and outcomes accurately within the EMR system.
Collaborate with care teams, facility staff, and referral partners to ensure continuity of care.
Maintain compliance with Medicare guidelines, state regulations, and clinical protocols.
Perform additional services within scope of licensure as agreed upon in contractor agreement.
