Job summary

Legacy Concierge seeks a dedicated Private Duty Registered Nurse to provide attentive 1:1 home care and hands-on clinical services. The role emphasizes direct patient care with mentorship, onboarding, and a defined clinical career pathway, ensuring continuity and trust during each shift.

Clinically supervised, the RN will complete onboarding, demonstrate patient-centered care, and work in a 1:1 private home environment per individualized plans of care.

Qualifications

• Active California RN license in good standing.
• Graduate of accredited ADN or BSN nursing program.
• ADN grads – minimum 2 years hospital experience; preferably ER/urgent care.
• BSN grads – minimum 8–12 months hospital or skilled nursing facility experience; preferably ER/urgent care.
• Current BLS/CPR certification.
• Ability to work independently in a 1:1 private care environment.
• Strong communication, documentation, professionalism, and customer service skills.

Responsibilities

• Deliver 1:1 skilled nursing care in the patient's home.
• Perform comprehensive head-to-toe assessments including physical, neurological, respiratory, cardiovascular, skin, and musculoskeletal status.
• Monitor, administer, and document vital signs during the shift.
• Administer medications safely and accurately per plan of care.
• Support ADLs and IADLs including toileting, dressing, feeding, meal prep, transportation, and companionship.
• Perform G/J-tube feeding and medication administration as ordered.
• Provide condition-specific management as directed by the plan of care.
• Perform basic wound care and dressing changes.
