Adoration Health in Nashville, TN is seeking a Private Duty Licensed Practical Nurse to join our team for 1:1 patient care in clients' homes. You will provide comprehensive nursing support, follow the care plan, and collaborate with families and case managers to ensure quality outcomes.

The role requires an LPN/LPT/LVN license, CPR certification, and a valid driver's license. Schedule includes days and overnight shifts with stability and growth opportunities within a growing home health company.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice.
• Valid driver's license and proof of car insurance.
• CPR certification required.
• 1+ years of nursing experience preferred.
• Home health care experience preferred.

Responsibilities

• Provide 1:1 care to follow client care plan while maintaining dignity and independence.
• Administer medications, feedings, oxygen, ostomy care as needed and assess results.
• Document nursing actions and communicate with family, case manager, and physicians.
• Coordinate or order supplies, medications, and feedings to ensure availability.
• Ensure all patient needs are met per physician instructions.
