H2 Health Prime Living in The Villages, FL seeks a Physical Therapist to provide 1:1 care in outpatient senior living settings. You will create personalized treatment plans and collaborate with a multidisciplinary team to restore function and improve quality of life.

Qualifications

• DPT degree or equivalent from an accredited PT program.
• Active or eligible state PT license.
• Prior outpatient experience a plus but passion for patient care valued.

Responsibilities

• Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
• Design and implement personalized treatment plans to restore function and improve quality of life.
• Document patient progress accurately using EMR.
• Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
• Maintain a positive, professional clinic environment reflecting company culture.
