Job summary

H2 Health Prime Living in Stuart, FL seeks a Physical Therapist to provide 1:1 care in a luxury senior living community. Flexible Part-Time (10-15+ visits/week) or Full-Time (35+ visits/week) schedules with a patient-centered focus.

Qualifications

• Degree from an accredited Physical Therapy program.
• Active or eligible PT license to practice in the state.
• New graduates welcome; outpatient experience is a plus.

Responsibilities

• Evaluate and treat patients in an outpatient setting.
• Design and implement personalized treatment plans.
• Document patient progress using EMR.
• Collaborate with a multidisciplinary team for holistic care.
• Maintain a positive, professional clinic environment.
