Job summary

Interim HealthCare Inc. is seeking a Private Duty Nurse (PDN) LPN to provide 1:1 skilled nursing care for a pediatric patient at home. The role emphasizes consistent scheduling, overnight shifts, and daytime options, with a focus on safety and compassionate, individualized care.

You will document care, monitor vitals, administer treatments, and collaborate with family and the care team to ensure seamless continuity of care. CPR/BLS certification and pediatric nursing experience are required.

Qualifications

• Graduation from an accredited practical (vocational) nursing program.
• Current, unrestricted LPN license in the state of Ohio.
• Pediatric nursing experience is required; one-on-one or home health care experience is a plus.
• Current CPR/BLS certification.
• Reliable transportation to and from the patient's home.
• Strong assessment, communication, and documentation skills.
• Ability to work independently while following the plan of care.

Responsibilities

• Provide 1:1 in-home skilled nursing care to a pediatric patient according to the plan of care.
• Monitor and document the patient's condition, including vital signs and changes.
• Administer medications and treatments as prescribed, documenting care.
• Assist with personal care and daily needs as outlined in the plan of care.
• Monitor for illness, infection, respiratory concerns, or changes, and report promptly.
• Provide compassionate, age-appropriate care while promoting comfort and safety.
• Communicate with family, supervising RN, providers, and office team to ensure continuity of care.
• Complete accurate clinical documentation and shift handoffs.
