Job summary

Trinity Health is seeking a Patient Safety Observer to provide continuous, direct visual monitoring for patients requiring 1:1 observation. You will remain with the patient at all times, quickly reporting changes in behavior or safety concerns to the RN, while maintaining dignity and confidentiality.

In this non-clinical position, you will support the care team by observing for potential hazards, following suicide-prevention procedures, and adhering to hospital policies.

Qualifications

• Must observe patients continuously and report safety concerns promptly.
• Remain with the patient at all times and never leave unattended.
• Follow hospital suicide-prevention procedures and safety policies.
• Complete required training in suicide prevention and patient safety.

Responsibilities

• Maintain continuous, uninterrupted visual observation of the assigned patient.
• Remain with the patient at all times and never leave the patient unattended.
• Observe behavior, activity, surroundings for potential safety concerns.
• Immediately notify the RN of suicidal statements, self-harm attempts or safety concerns.
• Follow all safety precautions and hospital policies.
• Provide basic, non-clinical comfort and assistance as directed by nursing staff.
• Maintain confidentiality and professional boundaries.
• The RN retains responsibility for clinical assessment and interventions.
