Job summary

Concord Hospital in New Hampshire is seeking a Patient Safety Attendant to provide 1:1 monitoring for behavioral health patients and those at risk of self-harm or delirium. Under supervision of a Registered Nurse, LPN, or Paramedic, you will observe, document, and promptly alert the team to intervene as needed.

This role requires empathy, calm communication, and strict adherence to safety policies. Shifts include days, nights, weekends, and holidays, with training provided for Heart Saver and

Qualifications

• High School Diploma or GED required.
• Heartsaver certification within 6 months of hire.
• Aggressive Behavior Management training within 6 months of hire.

Responsibilities

• Provide 1:1 monitoring with continuous observation for patients at risk of self-harm, falls, delirium, or behavioral health concerns.
• Identify risks and promptly alert the team to intervene.
• Document all pertinent information and maintain patient safety protocols.
• Maintain infection prevention standards and confidentiality of patient information.
• Assist with patient mobility, meals, and daily living activities as directed by RN/LPN/Paramedic.
• Remain with the patient during 1:1 observation until relieved by another staff member.
• Communicate effectively with patients, families, and the healthcare team.
